SHARP Enrollment & Back To School Itinerant Family Workshop

Saturday, August 19th, 9:00am-2:00pm at FBC 1234 E. Northern Ave. Phoenix, AZ 85020 - Please join us for a fun filled family workshop and register for SHARP! Free event, pizza lunch will be provided.

Please note that SHARP is an independence program. We've established the following criteria for program participants. Please select all that apply to your child.(Required)

My child has a documented visual impairment

They attending grades kindergarten through high school

They are able to eat, toilet, dress independently

Able to communicate independently via verbal, sign or communication device

Able to actively participate in a group setting without negatively impacting other participants. Inappropriate behaviors include: Running away, physical aggression, displays of consistent defiant behavior

Does not require constant supervision by a qualified health aide or nurse
